Standardized diagnostic criteria are essential for differentiating migraine subtypes in clinical practice
1. Migraine Without Aura (ICHD-3 1.1)
Requires at least 5 attacks lasting 4–72 hours (untreated or unsuccessfully treated)
Unilateral location
Pulsating quality
Moderate or severe pain intensity
Aggravation by or causing avoidance of routine physical activity
Additionally, the attack must include at least 1 of the following associated features
Nausea and/or vomiting
Photophobia and phonophobia
2. Migraine With Aura (ICHD-3 1.2)
Requires at least 2 attacks accompanied by fully reversible aura symptoms (visual, sensory, speech/language, motor, brainstem, or retinal)
Key aura characteristics include gradual spread over $\ge 5$ minutes, individual symptom duration of 5–60 minutes, unilateral presentation, and headache onset within 60 minutes of aura resolution
